Thank you for using the NOAH 3 Questionnaire module.
Version 1.1 of the Questionnaire Module contains the COSI and APHAB questionnaires, in English and German.
Additional standard questionnaires or additional languages may also be added to future versions of this module by HIMSA, based on availability and the needs of NOAH 3 users.
NOAH 3 Requirements
The NOAH 3 Questionnaire Module requires:
- NOAH 3.1.2 with Patch D or later.
- Internet Explorer 5.5 or later, installed as a "typical installation". For more information, see the "Known Issues" section below.
How to Install the NOAH Questionnaire Module
Make sure that NOAH System is shut down.
Download the Questionnaire module and double-click on the download file. Then follow the instructions.
When you start NOAH 3, the Questionnaire Module icon will appear in the "Module Selection" window under the "Other" tab.
Known Issues
Error 805104: "Unable to register module" message when installing on a NOAH System 3 network client Problem: Some NOAH System users may receive the following error when attempting to install the Questionnaire Module on a NOAH 3 network client:

Solution: For NOAH System, upgrade your network server to version 3.1.2 patch D (or later) and try installing the Questionnaire Module again. NOAH 3 Patches and updates are available for download on the HIMSA download site.
Error 805103: "Unable to register module" message when installing on a NOAH 3 based Office SystemProblem: Some NOAH 3 office system users may receive the following error when attempting to install the Questionnaire Module:
Problem: Unable to register module. Solution: ... Error number: 805103 |
Solution: Your office system may need to be updated. Contact your office system vendor.
Display problems caused by missing Internet Explorer component
Problem: The Questionnaire Module requires an Internet Explorer component that is installed with Internet Explorer 5.5 and above. If you have an earlier version of Internet Explorer, or if you have installed Internet Explorer as a "Minimum Installation", this component may be missing.
Without this component, the Questionnaire Module will not display or function correctly. You may also receive the following error:
"To display this page correctly, you need to download and install the following components: Vector Graphic Rendering (VML)."
Solution: If you receive the message above, download and install the VML file. Otherwise, you can also solve the problem by installing Internet Explorer 5.5 or later using the "Typical" installation method.
Then reinstall the NOAH Questionnaire module.
Windows 98 SE, Error 80070216, when printing questionnaires
Problem: The Questionnaire Module displays an error number 80070216 when attempting to print.
This error is very rare. It is caused by another program, such as a printer driver or a spyware detector, changing the default "Floating Point" configuration setting that is used by the Questionnaire module. If the other program does not return the setting to its default value as required, the Questionnaire module cannot print.
Solution: The only solution is to find the software or driver which is creating the error. In some cases, it may be enough to update the software or driver to a newer version. In other cases, it will be necessary to uninstall the software.
