The following instructions describe how to download and install the NOAH Questionnaire module into NOAH System 3.

| The NOAH 3 Questionnaire Module requires NOAH 3.1.2 with Patch D or later. This module cannot install under Noah 4. |
The NOAH Questionnaire module for download from the table below is Version 2.0. Version 2.0 includes APHAB questionnaires in English, German, Hungarian and Portuguese (Brazil), and COSI questionnaires in all NOAH Languages.
Download and install the NOAH Questionnaire module as follows:
- Click on the link below. The 'File Download' window appears. Click 'Save this program to disk', then click 'OK'. (Some systems require you to click 'Save' only.)
- The 'Save As' window appears. Using the pull-down menu, locate a folder in which to save your file. (You can choose any location.) Click 'Save'. Download begins.
- When download is complete, double-click on the file you have just downloaded and follow the installation instructions.
- If you wish to install the module on another NOAH 3 computer, copy the file "NOAH Questionnaire.exe" onto a CD, transfer the file to the new computer and follow the installation instructions in the Readme file.
