The NOAH Questionnaire Module allows NOAH 3 users to complete, store and retrieve industry-standard questionnaires to assist in the provision of evidence-based hearing health care. Version 2.0 includes the COSI questionnaire (in all NOAH languages) and the APHAB questionnaire (in English, German, Hungarian and Portuguese-Brazil).

The Questionnaire Module is a free add-on for registered NOAH 3 users (version 3.1.2 Patch D or later).
